Risk of facial splashes in four major surgical specialties in a multicentre study

本研究は、顔面の防護を行わない場合の曝露リスクの判定を目的とした、6カ所の医療施設でのフェイスシールドへの血液のはね返り汚染の分析である。血液のはね返り曝露を、全発生率、フェイスシールド上のはね返りの部位、外科の専門科、手術室スタッフに対するリスク、手術所要時間、および出血量に基づいて評価した。視覚的検査およびロイコマラカイトグリーンによる染色法による血液のはね返り汚染を、フェイスシールド600個で評価した。フェイスシールドを眼窩部（O領域）、眼窩周辺部（P領域）、およびマスク部（M領域）の3領域に区分した。視覚的検査では、フェイスシールドの50.5％（303/600）に血液のはね返り汚染が検出されたが、ロイコマラカイトグリーンによる染色法では66.0％（396/600）に血液汚染が検出された。O領域では36.6％（220/600）、P領域では37.8％（227/600）、M領域では57.0％（342/600）に血液汚染がみられた。手術室スタッフ別にみると、血液のはね返りの発生率は執刀医が83.5％（167/200）で最も高く、第一助手の68.5％（137/200）、手術室看護師の46.0％（92/200）がこれに続いた。専門科別では、心臓外科の発生率が75.3％（113/150）で最も高リスクであり、神経外科の69.3％（104/150）、消化器外科の60.0％（90/150）、整形外科の60.0％（90/150）がこれに続いた。

HCVは結膜感染の事例もあるため、目の保護は職業感染予防策の観点から重要である。古典的な手法であるが、現実を知る上ではためになる論文である。
