肝胆膵癌手術後の血糖コントロール不良により手術部位感染が増加する:手術件数の多い日本の医療施設における前向き研究★

肝胆膵領域手術の待機患者連続265例の手術部位感染(SSI)について前向きに観察した。手術部位感染率は肝胆膵手術の種類により異なっていた。多変量解析により、腸管吻合術、術後の血糖コントロール不良、および癌の種類が独立した危険因子であると判定された。手術部位感染率は、術後期間中に発現した高血糖の程度と直接的な相関があった。特に、インスリン注入療法により血糖値が200 mg/dL未満に維持されていた患者の手術部位感染率は5/25(20%)であり、インスリン注入療法を行っているにもかかわらず血糖値が200 mg/dL未満に維持されていなかった患者の感染率 49/94(52%)より有意に良好であった(P<0.01)。手術部位感染率を低下させるためには、術後の血糖値を200 mg/dL未満に維持する必要がある。
