Meticillin-resistant Staphylococcus aureus in Norway, a low-incidence country, 2006-2010

背景
抗菌薬耐性は世界的な公衆衛生上の脅威である。ノルウェーでは耐性菌の発生率は、医療施設と市中のいずれにおいても低レベルに維持されている。メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の単一症例とアウトブレイクはともに報告が義務化されており、すべての分離株の遺伝子型判定が行わている。
目的
ノルウェーにおける MRSA の疫学について述べるとともに、発生率が低い国で MRSA がどのように伝播しているかを解析すること。
方法
ノルウェーで 2006 年から 2010 年に報告された MRSA 保菌・感染の検査確認例の全症例を対象として、疫学的解析を実施した。
結果
合計 3,620 例の MRSA 症例が特定された。このうち約 3 分の 1 は海外から持ち込まれたものであり、3 分の 1 は国内の医療施設、残りの 3 分の 1 は市中で感染したものであった。症例の 12％は既知のアウトブレイクと関連があった。保菌・感染患者の全発生数は徐々に増加していた。重度の感染症の症例数は年間約 20 例で安定しており、医療関連感染による MRSA 症例の割合は減少していた。
結論
MRSA はノルウェーの住人には現在もまれにしかみられず、病院や介護施設の細菌叢内での MRSA の常在化を予防するための戦略は妥当である。

監訳者コメント：

輸入感染症としての MRSA の位置づけがノルウェーでは明確化している。我が国とは分離頻度があまりにも違うが、こうした世界各国の耐性菌事情から学べるとこは多い。
