日本における消化管手術後のクロストリジウム・ディフィシル(Clostridium difficile)関連疾患の負荷
The burden of Clostridium difficile-associated disease following digestive tract surgery in Japan

背景
手術にはクロストリジウム・ディフィシル(Clostridium difficile)関連疾患(CDAD)のリスクがあると考えられているが、術後 CDAD の転帰に関する大規模なデータはほとんどない。
目的
日本の DPC(診断群分類)入院患者データベースを用いて、消化管手術後の CDAD 発症に関与する因子および CDAD の転帰を解析した。
方法
2007 年から 2010 年に癌に対する食道切除術、胃切除術、および大腸切除術を受けた患者から、術後に CDAD と診断された患者を特定した。患者背景と病院の因子を補正し、CDAD 発症率と院内死亡率についてはロジスティック回帰分析、術後入院期間および総費用については多重線形回帰と傾向スコアを 1 対 1 でマッチさせた解析を実施した。
結果
消化管手術を受けた 143,652 例の患者から 409 例(0.28%)の CDAD 患者を特定した。Charlson 併存疾患指数高値、長期の術前入院期間、および非大学病院が CDAD 発症率高値と有意に関連していた。院内死亡率は、CDAD 患者のほうが非 CDAD 患者と比較して高値であった(3.4%対 1.6%、オッズ比 1.83、95%信頼区間[CI]1.07 ~ 3.13、P = 0.027)。CDAD に起因する術後入院期間および CDAD 関連コストは、線形回帰ではそれぞれ 12.4 日(95%CI 9.7 ~ 15.0、P < 0.001)、6,576 米ドル(95%CI 3,753 ~ 9,398、P < 0.001)、傾向スコアをマッチさせたペア解析ではそれぞれ 9 日(P < 0.001)、6,724 米ドル(P < 0.001)であった。
結論
高死亡率、長期入院、および高費用が術後 CDAD と関連していた。これらの結果は、消化管手術を実施した患者にはさらなる CDAD 制御策が必要であることを示している。

日本からの発表である。解析の対象を食道切除術、胃切除術、および大腸切除術を受けた患者から、術後に CDAD と診断された患者に限定しているのが本論文の特徴である。国内事例では一般的な長期入院者・抗菌薬投与群、高齢者、経管栄養実施者、呼吸管理者などがおそらく影響因子となっているが、この論文では評価されていない。
