Challenges of controlling a large outbreak of OXA-48 carbapenemase-producing Klebsiella pneumoniae in a French university hospital

Nantes University Hospital における OXA-48 カルバペネマーゼ産生肺炎桿菌（Klebsiella pneumoniae）の大規模なアウトブレイクについて調査した。初発症例は渡航歴および海外での入院歴はなく、2013 年 6 月に流行株が尿サンプルから分離された時点では内科部門に 1 か月以上入院していた。週 1 回の消化管保菌のスクリーニングにより、二次症例 72 例がアウトブレイク中に検出され、2013 年の 6 月から 10 月（33 例）と 2013 年 11 月から 2014 年 8 月（39 例）の 2 相性を示した。流行株拡散の原因は感染症部門（32 例）と内科部門（26 例）が近接していること、および両部門間の職員の移動であり、腎移植部門においても感染症部門からの曝露患者 1 例の移送後に二次症例 14 例が認められた。大半の患者（90％）は保菌であり、流行株に関連する死亡はなかった。3,000 例以上の接触患者を再調査し、6,000 件の直腸スワブを実施した。初発症例の発見が遅れたため、初期の制御策ではアウトブレイクを制御できなかった。制御策の成功が不完全であった原因は、3 期間にわたるコホーティングの実施遅延、病棟間の多数の移送、および症例の頻回の再入院などであった。

保菌者の再入院や病棟間移動といったコントロールの難しい背景で発生したアウトブレイク事例のレポートである。感染拡大の一因としてコホーティングの遅れが示されていたが、標準予防策の遵守状況が気になった。
