手術中の高濃度吸入酸素が手術部位感染に及ぼす影響:ランダム化対照試験のメタアナリシス

背景
手術部位感染(SSI)は著しく高い死亡率および罹患率の原因となる。手術を受けた患者への高濃度吸入酸素(FiO2)の投与は潜在的な予防戦略となる可能性がある。
目的
手術を受けた患者において、高 FiO2 と通常 FiO2 とを比較したランダム化対照試験のメタアナリシスを実施し、高 FiO2 が SSI の発症に及ぼす影響を推定すること。
方法
全身麻酔下で手術を受け、SSI が認められた成人を対象に高 FiO2 と通常の FiO2 を比較したランダム化対照試験(2015 年 12 月まで)の包括的な検索を実施した。
結果
本研究は、ランダム化対照試験 17 試験の患者 8,093 例を対象とした。感染率は、対照群で 13.11%、高酸素群で 11.53%であり、全リスク比は 0.893 であった(95%信頼区間[CI]0.794 ~ 1.003、P = 0.057)。また、国、SSI の定義、手術の種類で層別化したサブグループ解析でも、同様の結果が得られた。しかし、高FiO2 は結腸直腸手術を受けた患者に大きな利益をもたらし、リスク比は 0.735 であった(95%CI 0.573 ~ 0.944、P = 0.016)。
結論
手術、とくに結腸直腸手術を受けた患者への高 FiO2 の投与が SSI リスクを低減することを示唆するエビデンスは中程度である。介入へのより良好な遵守を伴うさらなる研究は、今回のメタアナリシスの結果に影響を及ぼす可能性がある。

監訳者コメント:
高濃度吸入酸素を利用した手術において SSI のリスクが変わる。しかも術式によってそのリスク比が異なることは驚きである。さらなる検証が必要だろう。
