Late-onset sepsis due to Staphylococcus capitis ‘neonatalis’ in low-birthweight infants: a new entity?

超低出生体重児の 5 人に 1 人が入院中に敗血症を発症する。早期産児におけるスタフィロコッカス・カピティス（Staphylococcus capitis）による敗血症の出現は最近になって観察された。本研究の目的は、早期産児における S. capitis 関連敗血症の臨床的重症度を評価することであった。コアグラーゼ陰性ブドウ球菌による敗血症を発症した乳児 105 例のうち、74 例が S. capitis によるものであった。重症の乳児は、S. capitis 以外のコアグラーゼ陰性ブドウ球菌群（32.0％）よりも S. capitis 群の方が多かった（55.4％）（P ＝ 0.03）。多変量解析の結果、S. capitis 関連敗血症が、重症であることの独立したリスク因子であることが示された。

監訳者コメント：
S. capitis はコアグラーゼ陰性ブドウ球菌（CNS）の一つであるが、従来の生化学的性状による同定は信頼度が低く、このため本菌の病原性と感染症に関する臨床的な特徴ははっきりしてはいなかった。本検討では、MALDI-TOF による同定を行い、他のCNS との比較を試みている。新たな疾患概念かどうかは今後の検討を待たねばならないが、非常に興味深い知見である。
