VIM-carbapenemase-producing Escherichia coli in a residential care home in The Netherlands

背景
カルバペネマーゼ産生腸内細菌科細菌（CPE）は、公衆衛生上の重大な脅威であり、その脅威は増大している。病院や長期ケア施設において、伝播予防のために保菌者を特定すべきであるが、感染制御のためのガイドラインが、あらゆるタイプのケア施設に適用できるとは限らない。
目的
オランダの居住型ケア施設（入居者は個室付きの集合住宅に居住するが、共有設備も使用する）における VIM 型メタロβ‐ラクタマーゼ産生大腸菌（Escherichia coli）のアウトブレイクの調査について報告すること。
方法
保菌率および定着率を評価するために、接触・環境に関する巡回スクリーニングを実施した。施設の内部特性により、特別の感染制御策が必要であった。接触予防策、衛生状態の改善、教育などの介入バンドルを実施した。
結果
全体で、居住者 110 例のうち、発端症例を含む CPE 保菌者 8 例が特定された。VIM 型 CPEの拡散は、共有領域の共用トイレの使用と関連した。最初の症例の発見から 7 か月後に全保菌者が VIM 陰性であることが確認され、1 年後、VIM 型 CPE は環境内にもはや検出されなかった。
結論
アウトブレイクの制御を達成するために特別なバンドルアプローチが必要であった。多剤耐性病原菌の拡散に効果的に取り組むために、現行のガイドラインは、あらゆるタイプの居住型ケア施設に合わせて変更すべきである。

ケア施設における耐性菌の拡散、蔓延は我が国でも心配される課題である。日本においても同様な検討が必要と考える。
